Diabetes Self-Management Education Program
Diabetes touches more lives than most people realize. According to the Centers for Disease Control, 29 million people in the United States (9.3 percent) have diabetes. Its diagnosis increases every year in adults and children – and many people who have it don't even know it.
To help manage this community crisis, DeKalb Health offers a Diabetes Self-Management Education Program led by team that includes a registered dietitian, registered nurse, certified diabetes educator, pharmacist, exercise physiologist and a medical social worker. The program staff works closely with the patient’s family physician to ensure the best outcomes.
